Delmar Office: New Patient Forms for Your First Visit
For our new Delmar medical patients: Before you arrive for your first visit, we ask that you complete the new patient information in our Electronic Medical Record (EMR) and bring with you the signed Consent Forms (please print the consent forms located below). As we are an integrative practice and you may be seen by more than one practitioner at some point in your treatment, there are additional consents that we ask you to sign initially.
Please use the access code given to you at the time you made your appointment to access our EMR system through our Patient Portal.

Integrative Medicine is a patient centered approach to healthcare that attends to the whole person- body, mind, heart and spirit. We consider the relationship that develops between client and practitioner an essential part of the therapeutic experience. This requires a commitment of time and effort from both parties. To begin this process we ask that you complete your patient information in the EMR before your visit. If you cannot use this system, please contact our office and we can send you forms or come to your visit 30 minutes before your scheduled time.
In addition, we would like to remind you to bring or fax any pertinent medical records or recent tests to this appointment. Thank you for your efforts. We look forward to your visit and serving you in your health goals.
New Patient Scheduling Policies:
As a reminder to our new patients, the payment you made towards the scheduling fee and electronic medical records generation/maintenance, at the time you scheduled your first medical appointment, is not refundable. We will deduct this payment to the cost of your initial appointment.
Please also know that we understand when other urgent personal matters interfere with keeping a scheduled appointment. We do require 3 business days prior to the date of your scheduled appointment, to reschedule; missed appointment fees may otherwise apply when appointments are cancelled /changed in less than the requested time. We want to be of service to all our patients, and respectfully request that t his time frame for changing appointments is adhered to so that we can refer to a healthy waiting list for those who are trying to be seen earlier. We appreciate your understanding.
