New Patient Forms for Your First Visit

Welcome to the Stram Center for Integrative Medicine,  where you benefit from the skill and knowledge of Dr. Stram and his whole team.

Integrative Medicine is a patient centered approach to healthcare that attends to the whole person- body, mind, heart and spirit. We consider the relationship that develops between client and practitioner an essential part of the therapeutic experience. This requires a commitment of time and effort from both parties. To begin this process we ask that you complete your patient information in the Electronic Health Record (EHR), under the Account tab,  before your visit. If you cannot use this system, please contact our office and we can send you our forms; we ask that these forms be received at least three days prior to your scheduled appointment.  These forms include a  complete medical history questionnaire, as  well as the consent forms and office policy agreements, under the Consents and Medical History Questionnaire tab of your portal.  As  are an integrative practice and you may be seen by more than one practitioner at some point in your treatment, therefore there are additional consents that we may ask you to sign. 

If you are unable to access our EMR through the patient portal, please let us know by calling our office: (518) 689-2244. Please remember that the link that was sent to you when you scheduled your  first appointment to create your portal access does expire in 5 days, therefore you may need a new link if you have waited past this time frame to set up your account access.

In addition, we would like to remind you to bring or fax any pertinent medical records, blood work or recent tests to this appointment. You may also upload these records for your doctor to review directly in the EHR, under the Labs and Documents tab.

Please note that your portal provides you access to confidential, hipaa compliant messaging so that you can stay in touch with our nursing care team, should you have questions or medical concerns, place orders for supplements, and access receipts and wellness plans. Our system is easy and user friendly!

New Patient Scheduling Policies:

As a reminder to our new patients, the deposit payment you made towards your first medical appointment, at the time you scheduled,  will be deducted from the cost of your initial consultation. Please note, if you cancel your appointment within the required three days, and you do not reschedule, you will be refunded your deposit less $25 for scheduling costs. 

Please also know that we understand  urgent personal matters,  sickness or inclement weather, can interfere with keeping a scheduled appointment. We do require 3 business days prior to the date of your scheduled appointment, to reschedule; missed appointment fees may otherwise apply when appointments are cancelled /changed in less than the requested time. The deposit you provided at the time of your appointment will be applied to this missed appointment charge. We want to be of service to all our patients, and respectfully request that this time frame for changing appointments is adhered to so that we can refer to a waiting list for those who are ill and trying to be seen earlier than their scheduled appointment. We appreciate your understanding.
